Appointments are typically scheduled through the front desk, however, you may need to speak with the nurse before scheduling the appointment in order to assess your needs.
Please arrive ten minutes before your appointment to complete any necessary paperwork to help us quickly process your claims.
You will need to bring your insurance cards with you to every appointment. You will be responsible for obtaining referrals before your appointment and paying your
co payment at the time of service. If you have any questions, contact your insurance carrier.
If you cannot keep your appointment, please notify our office as soon as possible.
