Salina Clinic will only release your medical information with your written permission or as required or allowed by Kansas Statute, unless the records are necessary for your treatment by another physician or in emergency situations. To request your medical information be sent to another physician, you will be asked to complete a special form authorizing the release of medical information. A fee may be charged for copying and sending your records.

If you are a new patient to the Salina Clinic please make arrangements to have your records transferred to our office before your appointment to aid us in providing top quality care to you.
